If you have a current DVA referral (D904) from your GP, you can book directly with us. Your physio sessions may include:

Frequently Asked Questions
You’ll need a current referral from your GP (D904 form). Once that’s in place, you can contact us to book an appointment. We’ll handle the claiming directly through DVA — no out-of-pocket cost to you.
Yes, please bring a copy of your DVA referral and your DVA card. If your GP has sent it to us directly, just let us know and we’ll confirm it before your appointment.
Each DVA referral includes up to 12 sessions. If more sessions are needed, we’ll work with your GP to arrange a new referral.
Our clinic has a single entry point that is suitable for disabled access. If you have mobility concerns, let us know when booking so we can assist as needed.
